Improving Health Care for Milwaukee’s Vulnerable Populations

Established in 2007, the Milwaukee Health Care Partnership is a public/private consortium dedicated to improving health care for low income, underserved populations in Milwaukee County, with the aim of improving health outcomes, reducing disparities and lowering the total cost of care. To help achieve these goals, the partnership instituted the Emergency Department Care Coordination Initiative that uses MyHealthDirect's web-based scheduling technology to help ED patients find and book an appointment with a safety-net clinic.
